DESCRIPTION:Are you or a loved one living with chronic conditions\, such as fatigue\, pain\, sleeplessness\, shortness of breath\, stress\, and emotional problems such as depression\, anger\, fear\, and frustration? SAGE is hosting a 6-week Chronic Disease Self-Management Workshop in conjunction with Westfield Regional Health Department. This FREE workshop will help people with any ongoing or long-term health conditions\, or their caregivers\, find practical ways to manage their symptoms\, improve quality of life\, and reduce healthcare costs.\n\n\n\n"This program is a fun and practical course" said Katie Kovar\, LSW\, Manager of Social Work and Community Outreach for SAGE Eldercare\, "It helps people with chronic conditions and their caregivers overcome daily challenges and maintain an active and fulfilling life." \n\n\n\nThe program will be held on Tuesdays\, October 18th through November 22nd from 10am   12:30pm at SAGE Eldercare\, 290 Broad St\, Summit.
